Medical Records Request from NWAFT

Summary

A Subject Access Request (SAR) is a request to an organisation (under the Data Protection Act 2018) for a copy of personal data held by that organisation. 

This online SAR Portal has been designed to assist in accessing medical records held by the North West Anglia NHS Foundation Trust. The Trust’s Access Services Department will manage this portal to ensure that requests are responded to and actioned as appropriate.

Some of our forms require relevant forms of ID that are required to access medical records, to see what is accepted please click here.

Before responding to SARs, there are exemptions that the Trust has a duty to consider, such as the duty to protect the confidentiality of ‘third parties’. More information regarding SAR exemptions can be found on the website of the Information Commissioner’s Office here.
